Community Transition Services Navigator

Salary: $15 – $25/hr
Description:
In conjunction with the Michigan Department of Health and Human Services, the Community Transition Services Navigator oversees the transition process for persons with disabilities currently admitted to a nursing facility in the counties of Clinton, Eaton, Ingham, and Shiawassee. The program assists with the securing of affordable and accessible housing, initial household needs and follow along. Program participants are primarily the elderly, have a range of disabilities and may have barriers such as financial issues and criminal backgrounds.
· Completion of initial interview and assessment
· Determine program eligibility
· Use of person-centered planning approach to develop service plans
· Coordinate all logistics of housing transition including securing housing, coordinate with movers, locate and secure furniture
· Complete monthly reimbursement reports
· Attend and participate in CTS meetings
· Provide follow up support to participants (per contract requirements)
· Service Coordination for additional services needed to live independently
· Develop and maintain working relationships with community resources
· Other duties as assigned.
Requirements:
· Bachelor’s Degree in Human Service, Social Work or closely related field or Community Health Worker Certification.
· A minimum of three years’ experience in the provision of health or social services.
· Personal experience with a disability and/or knowledge of disability related issues.
· Must successfully pass a criminal background check, sex offender registry, and DHHS Central Registry.
· Access to reliable transportation as position requires travel.
· Occasional evenings and weekends.
· Strong verbal and written communication skills.
· Computer skills, including Microsoft Word, Outlook, Excel, MS Teams, and data entry.

Departmental Analyst 9-P11

Description:
The individual in this role will primarily serve as the SSA-Ticket to Work (SSA-TTW) liaison and will be responsible for working in collaboration with the Bureau of Services for Blind Persons, Field Services Division (FSD) staff and the Vocational Rehabilitation (VR) electronic case management system (ECMS) to identify eligible VR cases for SSA reimbursement, tracking all Social Security activities. This position will be the liaison on all matters related to BSBP Ticket to Work assignments, including the sole point of contact within the Social Security Cost Reimbursement (SSCR) portal, verifying customer social security information accuracy, initiating appeals, and remedying any disputes or discrepancies related to SSA- TTW reimbursements. Additionally, the SSA- TTW liaison will provide recommendations for changes to enhance the reimbursement process, assist with the creation of procedures, and identify the best use of available tools to complete the Ticket to Work assignment utilizing the SSCR portal.
The individual in this role will also serve as the primary BSBP litigation coordinator assisting with all actions related to hearings, appeals, monitoring litigations, and processing subpoenas for BSBP programs including Business Enterprise Program hearings and Vocational Rehabilitation appeals. Secondary functions will include performing tasks related to supporting the BSBP budget, including maintaining a procurement card for use in the Administrative Services Division (ASD), as well as serving as a backup to the FSD as a procurement cardholder to support the securing of lodging for FSD customers, review and update coding structure, complete year-end closing and review, and follow and make updates to the record retention process.
Requirements:
Possession of a bachelor’s degree in any major.
Two years of professional experience, including one year of experience equivalent to the intermediate (10) level in state service.

Departmental Analyst 9 – 12
Educational level typically acquired through completion of high school and the equivalent of at least two years of full-time active-duty experience at or above the E-6 level in the uniformed services may be substituted for the education requirement.
